Anti Cyclic Citrullinated Peptide Antibodies (Anti-CCP)

Rheumatoid arthritis (RA) is a common rheumatic disease of uncertain etiology with a significant level of morbidity. Despite decades of study and the development of a series of classification criteria, the diagnosis of RA remains empirical and imprecise, particularly early in the course of disease. Because early initiation of disease modifying treatments can significantly improve long term outcomes for patients with RA, there is considerable motivation to accurately diagnose RA.

Historically, the use of Rheumatoid Factor as a diagnostic tool for RA has been and remains problematic. Unfortunately, the latex fixation assay lacks specificity, being positive in many patients with various chronic disease states. Although nephelometry, which also detected IgM anti-IgG RF, was technically more reproducible and easier to perform, it did not improve sensitivity or specificity for RA relative to latex agglutination.

Anti-CCP is a highly specific and remarkably sensitive test for Rheumatoid Arthritis, making it a powerful tool in the serologic assessment of RA.



Clinical Utility:

Rheumatoid Arthritis Early Marker: Anti-CCP has been reported to be highly specific (98% specificity for RA, compared to 84% by RF).

 Differentiating Seropositive RA from the arthritis of Hepatitis C with Rheumatoid Factor (RF) positivity.

Differentiating other entities that can resemble RA with a positive RF (iePolymyalgia  Rheumatica and Parvoviral Arthropathy).

Predictor of the disease course in early RA.
